Agreements & Terms
program purpose
✓ I understand that the Social Change Program in which I have enrolled is designed to teach youth in the areas of marketing, leadership and social change, assisting them to be able to successfully apply acquired knowledge and gain valuable experience.
✓ I understand that I will be learning about and developing such skills under the supervision of an experienced facilitator.
✓ I understand that it is up to me to determine how I use my new skill sets. I understand I am responsible for my own outcomes and success.
special commitment clause
✓ I understand that this program is offered in partnership between Create Change Academy (a social enterprise) and Create Change Foundation (a registered charity).
✓ I, the undersigned, do hereby understand that 100% of funds raised will go directly to Create Change Foundation, which exists to empower girls living in extreme poverty.
✓ I understand that the Social Change program is a sustainable and meaningful fundraising strategy, where we raise money for girls living in poverty in Ghana while investing in Canadian youth to become social change leaders.
✓ I understand that Create Change Foundation is a Canadian registered charity, and as such in exchange for my training, and to solidify my newly acquired skills, I will work to reach the fundraising objectives set out.
✓ I understand that my work within this program can forever change the lives of girls my age in Ghana living in extreme poverty, in addition to the benefits I will receive as a participant.
payments
✓ I understand that a $25.00 CAD administration fee is required to officially register to cover administration costs to provide official volunteer hours and other program benefits. This payment is only required should students wish to formally complete Module 1.
✓ I understand the administration fee is non-refundable.
✓ I understand that that the refundable administration fee is in place to ensure that participants who are not serious about the program do not waste administrative resources.
cancellation policy
✓ I understand that if I cancel or withdraw from the program I am not entitled to any refund for fees or funds donated.
✓ I understand the program may be canceled up to a day prior to the training start date due to low enrolment. I
✓ I understand that if I decide to cancel or withdraw from the program, I must do so in writing by sending an email to the program facilitator.
program requirements
✓ I understand that students must achieve 100% of the module fundraising objectives to receive full program benefits.
✓ I understand that students have 1 year once the program term ends to reach their fundraising objectives to receive the full program benefits (certificate/ diploma, volunteer hour certificate, etc.), unless otherwise agreed upon in writing.
✓ I understand that to progress in the program to the next term/stage, students are required to meet 100% of the fundraising objectives set out before them before the next module begins.
✓ I understand that students who fail to achieve the module fundraising goals not be permitted to progress to the next program stage.
confidentiality and class recordings
✓ I understand all recordings are the intellectual property of Create Change Foundation.
✓ I understand that what takes place during practice drills, mentoring and coaching scenarios to which participants will be exposed are strictly confidential.
✓ I understand I must maintain strict confidentiality regarding any practice drills, mentoring and/or coaching taking place during training and practice.
✓ I understand failure to maintain confidentiality may result in unwarranted and unfair defamation of character of participating students. This could cause irreparable harm to myself and my colleagues; and would seriously impair the effectiveness of this practice-based training program.
✓ While I am free to discuss workshops that focus on skills, I understand that I am required to maintain strict confidentiality regarding the specific scenarios to which I am both directly and indirectly exposed.
course attendance
✓ I understand in order to receive full credit for this course I am expected to attend class live, or listen back to the class recording for the missed class and submit a missed class form.
grievance policy
✓ I hereby agree to address any grievance(s) with my facilitator. Should I believe outside assistant is necessary to resolve my issue, I agree to split the cost of an impartial third party mediator as a last resort.
limit of liability
I EXPRESSLY UNDERSTAND AND AGREE THAT BOTH Create Change Academy and Create Change Foundation and its officers, employees, and independent contractors SHALL NOT BE LIABLE TO ME FOR:
(A) ANY DIRECT, INDIRECT, INCIDENTAL, SPECIAL CONSEQUENTIAL OR EXEMPLARY DAMAGES WHICH MAY BE INCURRED BY ME, HOWEVER CAUSED AND UNDER ANY THEORY OF LIABILITY. THIS SHALL INCLUDE, BUT NOT BE LIMITED TO, ANY PERSONAL INJURY, EMOTIONAL DISTRESS, LOSS OF GOODWILL, OR OTHER INTANGIBLE LOSS;
(B) I UNDERSTAND I AM ALWAYS RESPONSIBLE FOR MY OWN ACTIONS. I ASSUME ALL RESPONSIBILITY FOR ANY ACTION TAKEN IN RELATION TO AN ONLINE LECTURE, TEAM BUILDING OR PLANNED INITIATIVES AS WELL AS ALL OUTCOMES AND RESULTS.
(C) IN THE EVENT THAT I DIVULGE INFORMATION THAT THE FACILITATOR DETERMINES INDICATES THAT I AM AT RISK TO MYSELF OR OTHERS, I UNDERSTAND THE FACILITATOR IS LEGALLY RESPONSIBLE TO NOTIFY THE APPROPRIATE AUTHORITIES. THE LIMITATIONS ON Create Change Foundation and its officers, employees, and independent contractors ABOVE SHALL APPLY WHETHER OR NOT Create Change Foundation and its officers, employees, and independent contractors IS NEGLIGENT OR HAS BEEN ADVISED OF OR SHOULD HAVE BEEN AWARE OF THE POSSIBILITY OF ANY SUCH LOSSES ARISING.
additional release of liability
✓ I hereby release Create Change Academy and Create Change Foundation and its officers, employees, and independent contractors of any and all liability associated with this program, education, promotional activities, or related activities.
ownership
✓ I understand Create Change Academy and Create Change Foundation jointly own the commercial intellectual property rights to all training materials produced, developed, and made available to participants through the Social Change Program.
✓ I understand that as a participant in the program, I must follow guidelines presented in how I may use Create Change Academy and Create Change Foundation's logos, names, photos, and other proprietary information.
✓ I understand participation in the Social Change Program, someactivities may involve audio and/or video recording for promotional, broadcast. By participating in the Social Change Program, I consent to recording and release, publication, or reproduction of said recordings. I release Create Change Academy and Create Change Foundation and its officers, employees, and independent contractors from any liability associated with such recording, use, and release of this material. I waive all rights to the material. I also waive any claims for payment in relation to said materials.
✓ I understand in all cases, utmost care will be taken to protect my confidentiality. My name and contact information are strictly confidential and will not be used in connection with any such materials or recordings.
non-discrimination policy
✓ Create Change Academy and Create Change Foundation does not exclude, deny benefits to, or otherwise discriminate against any person on the basis of age, race, creed, colour, national origin, sexual orientation, or disability in admission to, participation in, or receipt of the services and benefits under any of its programs and activities, whether carried out by Create Change Foundation directly or through a contractor or any other entity with which Create Change Foundation arranges to carry out its programs and activities.
